Job Description (JD):

• Provide direct patient care under the supervision of a Registered Nurse (RN) or physician

• Monitor and record vital signs, patient symptoms, and overall condition

• Administer medications and treatments as prescribed

• Assist with wound care, catheter care, IV therapy (as permitted by state regulations), and injections

• Perform patient assessments and report changes in condition promptly

• Support patients with activities of daily living (ADLs), including bathing, dressing, and mobility

• Maintain accurate and timely documentation in electronic medical records (EMR)

• Educate patients and families on care plans, medications, and health management

• Collaborate with the healthcare team to ensure coordinated, patient-centered care

• Follow infection control, safety, and compliance standards at all times
